Indications

Candida species cause the majority of fungal blood stream infections. These organisms are detected through routine aerobic/anaerobic blood cultures with NO ADDITIONAL PROCEDURES required for their isolation. Please refer to Blood Culture Aerobic/Anaerobic (Adult) or Blood Culture Aerobic/Anaerobic (Paediatric)

Malassezia species

Cultures for Malassezia species may be considered in patients within the NICU/PCCU with appropriate risk factors (central line, parenteral lipid therapy) and negative routine aerobic/anaerobic blood cultures. Alternative collection procedures are required for the isolation of this organism; please contact the Microbiology Laboratory to obtain the appropriate collection device (an Isolator 1.5 ml tube).

Blood (1.0 to 1.5 mL) should be collected directly into the Isolator tube received from the Microbiology Laboratory.

Histoplasma capsulatum, Blastomyces dermatitidis, and Coccidioides immitis

If there is clinical suspicion for a disseminated fungal infection due to Histoplasma capsulatum, Blastomyces dermatitidis, or Coccidioides immitis, please contact the Microbiologist On-Call to arrange for collection.

Blood (6 to 10 mL) should be collected in a Green top (sodium heparin) vacutainer tube.

Critical Value:  
Interpretive Comments: Recovery of organisms depends mainly on volume of blood collected and frequency of collection. Timing is less important, particularly if cultures are collected before antimicrobial therapy.
Special Processing: For Histoplasma capsulatum, Blastomyces dermatitidis, and Coccidioides immitis, please include clinical history on the Public Health Laboratory Requisiton.
