Critical Information Required: A completed Public Health Laboratory requisition with the following information is required:
- suspected hantavirus pulmonary syndrome (HPS) or hemorrhagic fever with renal syndrome (HFRS)
- date of onset
- symptoms
- travel History
- exposure to rodents or their excrement
Storage & Shipment: The specimen must be refrigerated.

The specimen must be received within 24 hours of collection.
