5 mL Red top Vacutainer tube

Pediatric:
0-2 years: Lavender 1.0 pk., Red 0.5 pk
2-10 years: 2 mL Lavender top, 2 mL Red top
Collection Information: Specimen should be transported at room temperature, stored at 2-8C prior to testing.

For referred-in (non-LHSC) samples:

Requisition with patient demographics (HCN included)
Available pertinent familial history or information:
ethnic origin, known affected family members
patient transfused before sample drawn
patient planning a pregnancy
CBC results
Stained blood film reflecting the CBC results
A fax number to which the report can be sent
