Collection Information: Minimum volume of serum or Heparin or Citrate plasma required is 1.2 mL.
Authorization by On-Call Biochemist required.
Hospital for Sick Children’s requisition required to be completed by physician.
Requisition: Sick Kids: Therapeutic Drug Monitoring Requisition
Test Schedule: Send STAT by All Canadian Courier to Hospital for Sick Children (Sick Kids).
Test must be sent immediately. Include Hospital for Sick Children’s requisition and PaLM worksheet with instructions to fax and phone results to the VH Core Lab.

Samples can be received at VH or UH Core Lab but results will always be returned to VH Core Lab to be entered by technologist at VH.

Call Sick Kids (416-813-5906) and tell them there is a STAT specimen for Pentobarbital testing.

Send completed Sick Kids requisition, with work list. Ensure that phone and fax numbers for VH Core Lab are clearly written on the work list so results can be returned to the VH Core Lab.
