Critical Information Required: A completed Public Health Laboratory requisition with the following information is required:

Onset date
Outbreak number if applicable
Specimens from Day Care Outbreaks and children ≥30 days to < 4 years old are tested by a rapid ICT.
Specimens that are tested by ICT and are negative or positive will also be tested by EM.
Specimens from persons age < 30 days or 5 years and older are tested by EM only.
Storage & Shipment: The specimen must be refrigerated.
